Once you are admitted and enroll as a graduate student, it is important to be aware of required forms, upcoming deadlines, and pertinent information that will help you stay on track to graduate. The "Staying on Track" Checklist and deadline information below is designed to assist you with timely completion of required paperwork. It is not intended to replace or stand in lieu of the Graduate Catalog, your graduate advisor, or any university policies. It is your responsibility to remain familiar with the Graduate Catalog and your departmental expectations.
 
"Staying on Track" Checklist

  • Master's Degree Checklist
    • Prepare outline of curriculum with department head, graduate coordinator, or advisor, as appropriate for your department.
    • If applicable, remove conditional status with at least a 3.0 average after the completion of 12 hours of graduate credit. To remove conditional status, submit Petition for Regular Status form according to deadline below. The Admission to Candidacy form must accompany the Petition for Regular Status.
    • Encouraged to have advisory committee appointed prior to applying for admission to candidacy.
    • Apply for admission to candidacy after completion of 12 hours of graduate credit. Submit the Application for Admission to Candidacy form according to deadline below.
    • If applicable, file title of thesis at the time of admission to candidacy.
    • If applicable, complete language requirement.
    • Complete and submit the Graduation Check List form and Application for Degree form by the end of the first week of the last semester of graduate work. Refer to deadline below.
    • Pay diploma fee by the end of the first week of the last semester of study. Payable to UL Lafayette at the Student Cashier Center in Coronna Hall, room 102.
    • Order cap, gown, and hood from the UL Lafayette Bookstore. If desired, order graduation announcements and the like from UL Lafayette Bookstore.
    • Complete residence requirements. Refer to Graduate Catalog for details.
    • Take the written and/or oral exam as required and within the prescribed time limits provided below and within your departmental time lines.
    • Complete courses listed on outline of curriculum.
    • If completing thesis option, submit defended thesis to the Graduate School for format review. Pick up thesis for correcting edit suggestions and revisions.
    • If completing thesis option, turn in to the Graduate School office by deadline below four copies of the thesis in perfect, finished form, each with an approval sheet with original signatures in black ink, and two extra copies of the abstract with the appropriate heading, and pay thesis binding charge of $10.00 per copy.
    • Submit Separation Clearance form to your department.
    • Attend graduation exercises, unless excused in advance in writing by the Dean of the Graduate School.
       
  • Doctor of Philosophy Degree Checklist
    • Prepare outline of curriculum with department head, graduate coordinator, or advisor, as appropriate for your department.
    • Complete the Doctoral Advisory Committee Appointment form
    • If applicable, remove conditional status with at least a 3.0 average after the first 12 hours. To remove conditional status, submit Petition for Regular Status form according to deadline below.
    • Take and pass departmental preliminary examination(s).
    • Have dissertation committee approved by the Dean of the Graduate School. Submit the Doctoral Dissertation Committee Appointment form.
    • Complete courses listed on outline of curriculum.
    • Maintain a 3.0 average overall and in major field, with no more than one C in a course to be applied toward the fulfillment of degree requirements, and with the prescribed number of semester hours earned in courses at the 500 and 600 levels. Refer to the Graduate Catalog and your advisor for details.
    • Complete foreign language requirement, if applicable.
    • Satisfactorily complete the general comprehensive examination.
    • If applicable, present acceptable dissertation prospectus to dissertation committee.
    • Apply for admission to candidacy immediately after satisfactory completion of the general comprehensive examination. Submit the Application for Admission to Candidacy form according to deadline below.
    • Submit Doctoral Student Progress Report form by April deadline every year enrolled.
    • Complete and submit the Graduation Check List form and Application for Degree form by the end of the first week of the last semester of graduate work. Refer to deadline below.
    • Pay diploma fee by the end of the first week of the last semester of study. Payable to UL Lafayette at the Student Cashier Center in Coronna Hall, room 102.
    • Order cap, gown, and hood from the UL Lafayette Bookstore. If desired, order graduation announcements and the like from UL Lafayette Bookstore.
    • Complete residence requirements. Refer to Graduate Catalog for details.
    • Present dissertation in a form and according to a schedule acceptable to the dissertation committee before taking the final oral and/or written examination.
    • Take the written and/or oral exam as required and within the prescribed time limits provided below and within your departmental time lines.
    • Submit defended dissertation to the Graduate School for format review. Pick up dissertation for correcting edit suggestions and revisions.
    • Turn in to the Graduate School office by deadline below four copies of the dissertation in perfect, finished form, each with an approval sheet with original signatures in black ink, three extra copies of the abstract with the appropriate heading, one copy of the dissertation title page, and pay dissertation binding charge of $10.00 per copy.
    • Pay $83 fee for the service provided by ProQuest Information and Learning and $65 fee for copyright, optional, in the University of Louisiana at Lafayette Student Cashier Center in Corona Hall, room 102.
    • Complete the ProQuest Agreement form and survey of Earned Doctorate form and submit to the Graduate School office.
    • Submit Separation Clearance form to your department.
    • Attend graduation exercises, unless excused in advance in writing by the Dean of the Graduate School.
       
  • Doctor of Education Degree Checklist
    • Prepare outline of curriculum with department head, graduate coordinator, or advisor, as appropriate for your department.
    • If applicable, remove conditional status with at least a 3.25 average after the first 12 hours of graduate credit. To remove conditional status, submit Petition for Regular Status form according to deadline below.
    • Pass qualifying paper defense.
    • Have dissertation committee approved by the Dean of the Graduate School. Submit the Doctoral Dissertation Committee Appointment form.
    • Complete courses listed on outline of curriculum.
    • Maintain a 3.25 average overall and in major field, with no more than one C in a course to be applied toward the fulfillment of degree requirements, and with the prescribed number of semester hours earned in courses at the 500 and 800 levels. Refer to the Graduate Catalog and your advisor for details.
    • Present acceptable dissertation prospectus to dissertation committee.
    • Apply for admission to candidacy immediately after satisfactory completion of the qualifying paper. Submit the Application for Admission to Candidacy form according to deadline below.
    • Submit Doctoral Student Progress Report form by April deadline each year.
    • Complete and submit the Graduation Check List form and Application for Degree form by the end of the first week of the last semester of graduate work. Refer to deadline below.

Deadline Information
These deadlines are University and Graduate School deadlines; your department may have additional deadlines you must meet to satisfy program requirements. Check with your department to make sure you know what these departmental deadlines are. These deadlines represent a genuine effort to provide accurate information in effect at the time of posting; the University and the Graduate School reserve the right to make changes in the deadlines without notice.

Review Guidelines for Graduate Theses and Dissertations
You are encouraged to review the guidelines the semester prior to finishing your thesis or dissertation.
Submit final copies of Thesis or Dissertation (This deadline is for FINAL copies. You must submit a defended, committee approved copy earlier for Editor review. Your defense should be scheduled early enough to allow for committee-required and editor-required revisions to your thesis or dissertation.)
